Improved detection of ChatGPT confirmation and tool permission panels.

This version uses a more universal visible-card detector: it no longer depends only on specific button text such as “Confirm”, “Continue”, or “Execute”. Instead, it detects confirmation-style panels by their visible structure and selects the primary action button inside the active ChatGPT area.

Also improved performance for long chats by scanning only the visible part of the current chat instead of repeatedly scanning the full conversation history.
