This app gives you a form with general fields about your contact info and some job related questions such as "Years of Experience" or "Skills" but it doesn't let you customize your own fields. I was hoping I would to be able to autofill fields related to actual job experience (i.e. Workday forms) but it doesn't give you enough fields to do that.
